Hartford HealthCare Medical Group is proud to offer Medicare Annual Wellness Visits once every 12 months for patients with traditional and managed Medicare, at no cost to you.
A Medicare Annual Wellness Visit (AWV) is a yearly preventive wellness visit with your primary care team to create your personalized prevention plan. The visit will help your care team understand what is working for you and how to best support your continued health and well-being.
The visit includes:
- screening for depression, anxiety, safety risks and cognitive impairment
- review of medications, supplements and allergies
- review of medical, surgical & family history
- counseling for a healthy lifestyle
- establishing a Medicare screening and preventive services schedule
Your Appointment
Based on your Medicare plan, your Annual Wellness Visit will be conducted in-person or virtually by one of our specialized teams of advanced practice providers. These teams work in partnership with your PCP — they will not replace them. You will continue to see your PCP for all your care management.
In-Person Wellness Visits
In-person AWV for our Medicare and Medicare Advantage patients will be conducted by our dedicated Annual Wellness Clinician Team. They are well-trained for this type of appointment, and can spend extra time with you reviewing your health needs. The team may be available to see you either in your PCP’s office, or a nearby HHC office. Your PCP will review all notes from the visit, and schedule any necessary follow-up appointments.

Virtual Wellness Visits
We now offer virtual AWV for our Medicare and Medicare Advantage patients through our Expanded Virtual Health Team. This special clinical team will see you virtually (by video or phone), from anywhere across our network of care. Adult family members are also welcome to join and participate in the visits. Your PCP will review all notes from the visit, and schedule any necessary follow-up appointments.
