Services
Our Geriatrics Team will work with you to diagnose, treat and manage all of your unique and individual needs. We give special attention to situations of concern, including falls, incontinence, cerebrovascular disease, dementia, sensory impairment and other cognitive and affective changes that occur with aging.
Coordination of Care
Our Geriatrics Team collaborates with other Hartford HealthCare services in our network to assess and support families and caregivers in a number of ways. From our dementia specialists in our Center for Healthy Aging, to our skilled Hartford HealthCare at Home team, our entire system medical experts in all areas of health and wellness are available to you.
Inpatient Consultations
Our Geriatrics Team’s in-patient consultation program provides expert assistance in common geriatric problems, including delirium prevention and treatment, dementia with behavioral changes, medication management and end of life care. In-patient consultation are available at the following hospitals:
- Backus Hospital
- Hartford Hospital
- MidState Medical Center
- St. Vincent’s Medical Center
Outpatient Consultations
We have a number of primary care out-patient offices conveniently located across the state, dedicated to geriatric care. Our Geriatrics Team will conduct a comprehensive assessment to identify the signs of dementia and other geriatric conditions. Additionally, they will provide:
- Memory assessments
- Fall and balance evaluations
- Medication management
- Assistance with complex Geriatric syndromes
Resource for Elderly And Care at Home
Our REACH program works closely with your primary care provider to send an experienced advanced practice provider to your home for health visits. If you have difficulty getting to the doctor’s office, or are a high risk for hospitalization or readmission, REACH will provide you with the highest quality of care you need without leaving your home. Skilled Nursing Integration
Our Geriatrics Team of physicians and advanced practice providers will continue your care once entered into skilled nursing care at Hartford HealthCare’s Jefferson House, and at many other skilled nursing facilities in the Greater Hartford region, to ensure your wellness and prevent readmission to the hospital.
Community Partnerships/Geriatric Primary Care
Hartford HealthCare Medical Group has formed unique partnerships with Duncaster and Seabury senior communities in Bloomfield, providing comprehensive on-site skilled nursing and primary care for their residents, regardless of whether they are a current Hartford HealthCare patient.
