For Aetna, Anthem & United Managed Medicare Patients
Hartford HealthCare Medical Group is excited to now offer our Managed Medicare patients with Aetna, Anthem, and United (APN-CT) a specialized clinical team to conduct your annual wellness visits.
A member of this Annual Wellness Clinician Team will work with you to create and/or update your personalized prevention plan and perform health risk assessments. This informs your care team of your individual needs to support your highest level of health.
This team works in partnership with your primary care provider — they will not replace them. You will continue to see your PCP for all your ongoing care management and physical exams.
At your annual wellness visit, our clinician team member will:
- Review your medical history
- Update your medical record
- Review your current prescriptions
- Create a plan for recommended screenings
- Confirm advanced directives
All results from the annual wellness visit will be reviewed by your primary care provider, and be discussed with you at your next scheduled follow-up visit.
